Start up an operation for the first time; in a low-budget, accelerated mode; or after a long unplanned shutdown.
Require tasks to be completed in an expedient manner, especially right before the weekend or a major holiday. Believe that deadlines are real and immovable.
Permit a belief, among operators and operations management, that someone else is watching out for them (and assuring compliance to criticality controls).
Create criticality controls that can’t be readily confirmed. Don’t authenticate or verify controls.
Trust in tribal knowledge, historical "data", "skill of craft," and past experience and practices. Rely on memory.
Be lax about operations. Don’t label containers or fill out forms. Implement programs only so far as budget, priority, and time permit. Don’t fix or maintain equipment. Don’t self assess.
Don’t attempt to understand the process, how it really works, and how it breaks down. Don’t have Criticality Safety personnel talk to operators. Only provide oversight on an infrequent but predictable basis. Never observe back shifts.
Reinforce a culture that believes in the strength of its program because a criticality has never happened there (for thirty years).
Keep operators and operations management ignorant of criticality analysis, concerns, and controls.
Don’t train operators on criticality controls. Trust them to read and understand postings and procedures. Rely on their interpretation ability. Don’t use words and terms operators understand.
